The PREDIMED Trial Evidence and Its Correct Interpretation

The PREDIMED study enrolled 7,447 participants at elevated cardiovascular risk in Spain and compared Mediterranean diet groups against a low-fat control diet from 2003 to 2011. The results showed a 30 percent relative risk reduction in major cardiovascular events for the Mediterranean diet groups. Retracted and republished in 2018 due to randomisation issues at some sites, the corrected analysis produced similar conclusions. The anti-inflammatory effect was not attributable to any single compound but to a dietary pattern that simultaneously reduced pro-inflammatory omega-6 fatty acid intake, increased polyphenol consumption, eliminated refined carbohydrate spikes that trigger inflammatory cytokine release, and provided adequate fibre to support anti-inflammatory gut microbiome populations.

The 12 foods the evidence consistently supports: extra-virgin olive oil with minimum phenolic content of 250mg/kg, fatty fish at least twice weekly for EPA and DHA, blueberries and tart cherries for COX-2 inhibiting anthocyanins, walnuts for alpha-linolenic acid, leafy greens for lutein and nitrates that downregulate NF-kB, turmeric with black pepper for curcumin absorption, green tea for EGCG that reduces IL-6 and TNF-alpha, garlic for allicin derivatives, flaxseed for ALA and lignans, cooked tomatoes for bioavailable lycopene, legumes for Faecalibacterium prausnitzii prebiotic substrate, and dark chocolate above 85 percent cacao for flavonols that reduce CRP in multiple RCTs.

The Biomarkers Worth Tracking

High-sensitivity C-reactive protein is the most accessible inflammatory biomarker in standard blood panels. A reading below 1mg/L indicates low cardiovascular inflammation risk; above 3mg/L warrants investigation. Dietary interventions consistently show hs-CRP reduction within 8-12 weeks when adherence is genuine. The University of California San Diego found that a 12-week anti-inflammatory dietary intervention reduced IL-6 by a mean of 23 percent in participants with baseline levels above 3 pg/mL. Annual hs-CRP testing provides the most practical feedback loop for tracking dietary anti-inflammatory effectiveness over time.
